Were seeking a passionate and driven Physician Assistant/Nurse Practitioner to join our team in Caledonia, Michigan. This is an exciting opportunity to make a meaningful impact on the health of our community while working in a supportive, mission-driven environment.
Opportunity Details
- Outpatient Department of Trinity Health Grand Rapids Hospital
 - Our Caledonia location offers primary care services with a team of Providers that specialize in Family Medicine working together to deliver a comprehensive, personalized patient experience
 - Office also offers convenient on-site access to several services that are in the same facility including Lab, Advent Physical Therapy and Grand Rapids Ophthalmology
 - Practice currently has a team of 4 Physicians
 - 4 days/week schedule with extended hours and no weekends
 - No call for APP
 - Very close knit & collaborative office with a supportive environment

Requirements
- Primary Care experience required
 - APPs: Board certified in specialty prior to employment
 - Active Michigan medical license (or ability to obtain)
 - DEA license
 - ACLS/BLS certification (must be maintained)
 
Duties & Responsibilities
A typical day may include:
- Providing direct patient care
 - Collaborating with multidisciplinary teams
 - Participating in teaching and mentoring
 - Documenting in Epic EMR
 - Engaging in quality improvement initiatives
 - Supporting community health efforts
